The writ shall contain the name, place of abode and occupation of the plaintiff and of the defendant, so far as they can be ascertained, and the date (called “return day”) and place of hearing; and there shall be endorsed on the writ particulars of the claim signed by the plaintiff or his or her legal practitioner which shall state briefly and clearly the subject matter of the claim and the relief sought.
